Hello, and welcome! I'm Geraldine Taylor Sweeney, RN, BSN, founder and owner of The Check-In Diva Nursing Concierge.
Nursing is more than my profession, it's my calling. Since earning my associate degree in nursing in 2007, I have had the privilege of caring for patients during some of the most challenging moments of their lives. In 2011, I earned my Bachelor of Science in Nursing from California University of Pennsylvania, and I have continued to grow my knowledge and passion for helping others every step of the way.
As a proud wife, mother and lifelong caregiver, I understand the importance of compassion, trust, and family. My husband and I are blessed with three wonderful children and our grand cat who keeps us smiling with frequent visits.
In addition to caring for my clients, as an Infusion RN I also serve as an Adjunct Clinical Professor at a local university, where I enjoy mentoring future nurses and sharing the importance of compassionate, patient-centered care.
The inspiration for The Check-In Diva Nursing Concierge came from something I witnessed repeatedly throughout my nursing career. Patients with chronic health conditions would leave the hospital or rehabilitation facility with pages of discharge instructions, new medications, and follow-up appointments—but without the ongoing support they truly needed. I saw how overwhelming that transition could be, not only for patients but also for their families. Too often, questions went unanswered, concerns grew, and people felt like they were navigating their healthcare journey alone.
I knew there had to be a better way.
That's why I created The Check-In Diva Nursing Concierge—to be a trusted nurse, advocate, and partner for individuals living with chronic illnesses such as heart disease, cancer, chronic obstructive pulmonary disease, stroke, and type 2 diabetes after leaving hospital or rehabilitation centers. My goal is to provide personalized support, education, encouragement, and peace of mind between doctor's visits, helping clients feel confident, cared for, and empowered to live as independently and safely as possible.
I believe that every person deserves to feel heard, respected, and supported. Whether it's answering questions, coordinating care, checking in after a hospital stay, or simply being a reassuring voice when concerns arise, I am committed to treating every client like family.
